Irish Setter Genetic Registry GTA Application
 This form is to be used to register dogs that are the offspring of a blood tested sire and dam who are registered with ISGR or are of blood tested ancestry of dogs who are registered with ISGR.  This form is to be used only for dogs whose parents have DNA or GTA numbers issued by Irish Setter Genetic Registry.

Instructions for filing a GTA application:
 Effective August 1, 2004, no Irish Setters can be registered with the Irish Setter Genetic Registry unless they have one of the following types of information on their dog to be registered. All dogs already registered with ISGR are not affected.

 1. DNA Identification Certificate from AKC- copy sent with application
 2. Microchip : Must include type of microchip & number
 3. Tattoo: Must include number used
 

  1. Fill out and sign application.
  2. Submit a legible, 4-generation pedigree of the dog for which application is being made.
  3. Submit a copy of the dog's AKC registration certificate.
  4. Enclose application fee of $7.50 payable to OFA.. 
  5. Send application, pedigree & check to:
