Irish Setter Genetic Registry DNA Application
This form is to be used
to register dogs that have been blood tested by Optigen and found to be
genetically clear..
Effective August 1, 2004, no Irish Setters can be registered with the
Irish Setter Genetic Registry unless they have one of the following types
of information on their dog to be registered. All dogs already registered
with ISGR are not affected.
1. DNA Identification Certificate from AKC- copy sent with application
2. Microchip : Must include type of microchip & number
3. Tattoo: Must include number used
Instructions for filing a DNA application:
1. Fill out and sign this application. 2. Submit a legible, 4-generation pedigree of the dog for which application is being made
and identification information (above).
3. Enclose the application fee of $7.50 payable to OFA.
4. Submit a copy of dog's AKC registration 5. Submit a copy of the dog's Canine DNA Registry Certificate.
6. Send application, pedigree, check and DNA certificate to:

OFA. to publish this information. I agree
that certification records are open to inspection by any person so authorized by the ISGR. I agree that the ISGR, Inc. has the right to rescind any certifications for any reason, without refund of certification fees.
